As a participant in the 2025 Point in Time Survey, I understand that I may have access to sensitive information, including names and identifying information, address/location, personal financial information, and health information. I agree to maintain the security and confidentiality of survey participants and related information. I will not disclose, share, publish, copy or distribute any survey information without express permission.

Release
Upon completion of this electronic registration, I release and indemnify the organizers of the 2025 Point in Time Survey from prosecution or presentation of any claim for bodily injury or death or for property loss or damage incurred in connection with this survey project or related activities.
